Renew Your License
Current licenses expire at midnight, Eastern Time, on .
To ensure you receive your renewal notification from the department, your current mailing address must be on file. Failure to renew an active or inactive license by the expiration date will result in the license being placed in delinquent status. Failure by a delinquent licensee to renew before the expiration of the current licensure cycle renders the license null and void without any further action by the board or the department.
A licensee who remains on inactive status for more than two consecutive biennial licensure cycles and who wishes to reactivate the license may be required to demonstrate the competency to resume active practice by sitting for a special purpose examination or by completing other reactivation requirements.
Licensees requesting to reactivate their license from inactive or retired status are required to pay additional fees and comply with specific continuing education requirements. Please contact the board office at info@floridaspodiatricmedicine.gov to request your reactivation requirements.

The Department will renew your license upon receipt of:
- Completed renewal application
- Required fees
- Updated Practitioner Profile. To review and approve your practitioner profile, log into MQA Online Services and select ‘Review, Update & Confirm Profile’.
- Complete a mandatory background screening or retain* your current fingerprints. To learn more, please visit https://flhealthsource.gov/background-screening/.
*Required section 456.0135(6)(1), Florida Statutes, a $43.25 fee is required for FDLE to retain your fingerprint for background screening during this licensure cycle. This fee is collected at the time of licensure renewal if your fingerprint retention is expiring this licensure period. If your fingerprint retention expires in the next 60 days, you will need to pay separately through the Clearinghouse Applicant Initiated (CHAI) System.
Applicants for initial licensure must use a Livescan service provider to submit a set of fingerprints to the Florida Department of Law Enforcement (FDLE) for the purpose of conducting a search for any Florida and national criminal history records that may pertain to applicant. The results of the search will be returned to the Care Provider Background Screening Clearinghouse and made available to the Department for consideration during the licensure process. The fingerprints submitted by the applicant will be retained by FDLE and the Clearinghouse. All costs for conducting a criminal history background screening are borne by the applicant.
It is important to use the correct Originating Agency Identification (ORI) when submitting fingerprints. If you do not provide an ORI number or if you provide an incorrect ORI number to the service provider, the board office will not receive your fingerprint results, so it is extremely important to use the correct ORI when having your fingerprints scanned.
Applicants can use any FDLE approved Livescan service provider to submit their fingerprints. The applicant is fully responsible for selecting the service provider and ensuring the results are reported to the Department. If you are applying for initial licensure after December 15, 2012, You must print this form and take it with you to a Livescan service provider. For more information, FAQs, and a list of all approved Livescan service providers please visit the Department’s website at: FLHealthSource.gov/Background-Screening.
Renewal Instructions:
- Go to www.flhealthsource.gov and click the “Renew A License” button.
- Enter your Personal Identifying Information (PII), then click the Login button.
- Confirm or update your MQA Services Account email address, then click the “Continue” button.
- You are now in the Dashboard. During renewals, a “Renew My License” option will populate no later than 90 days prior to your license expiration date. (Note: If you do not see the “Renew My License” option, please check back at a later time.)
*** If the “PII Failed” screen displays, click the “Other Login Option” button and enter your existing User ID and password to access your account.***

- You may renew online if you have a credit or debit card to complete the transaction. Please note that you can now renew online, print a copy of your application summary and mail it with your cashier’s check or money order to:

Tallahassee, FL 32314-6320 - If you are renewing your license after the expiration date and your status still indicates Clear/Active, you will be assessed a delinquency fee. The delinquency fee will be in addition to your renewal fees.
- If your license was Delinquent/Active before the renewal deadline, it will be moved to Null and Void status after the renewal deadline. You should contact your board office for further instruction.
- The process of renewing a license may take 7-10 business days. Initiating contact to confirm the receipt of fees or the status of your license prior to this time will not expedite the renewal process.
- Note: Do not submit certificates of completion of in-service hours to the Florida Board of Podiatric Medicine.
If you are renewing online and need to change your name, please follow these instructions prior to renewing your license:
Name changes require legal documentation showing the name change. Please log into your MQA Online Services Portal account to submit your request and enter your full name as it appears on your legal documentation. If you are submitting your request online, please attach/upload supporting documents, which must be one of the following:
- A copy of a state issued marriage license that includes the original signature and seal from the clerk of the court
- A divorce decree showing the name change
- A court order showing the name change (Adoption, legal name change, federal identity change)
- A copy of a certificate of naturalization or H1B Employment Visa (Note: Foreign applicants and/or licensees may not have state issued documents)
Any one of these will be accepted unless the department has a question about the authenticity of the document.
NOTE: Please allow 5-7 business days for your request to be processed. If you are requesting a name change outside of renewal and wish to receive a new license reflecting the name change, please retrieve from the License Documents menu of your MQA Online Services Portal Account within two business days of the approval.
Certified check or money order should be made payable to the Florida Department of Health.

Licenses become Null & Void1 on . By submitting the appropriate renewal fees to the department, a licensee acknowledges compliance with all requirements for renewal, including continuing education.
A licensee who remains on inactive status for more than two consecutive biennial licensure cycles and who wishes to reactivate the license may be required to demonstrate competency to resume active practice by sitting for a special purpose examination or by completing other reactivation requirements.

Effective July 1, 2026: Pursuant to section 461.007(3), Florida Statutes, as amended by HB 1092, the two-hour continuing education course on the safe and effective prescribing of controlled substances is required only for podiatric physicians who are registered with the U.S. Drug Enforcement Administration (DEA) and authorized to prescribe controlled substances.

First Biennium Renewal
Pursuant to Rule 64B18-17.005, F.A.C., podiatric physicians initially licensed in the biennium or within 24 months of initial licensure are required to:
- Obtain five (5) hours of continuing education in the subject area of risk management. This may be obtained by attending one full day of a meeting of the Board of Podiatric Medicine at which disciplinary hearings are conducted, or by completing five (5) hours of Board-approved courses.
- If registered with the U.S. Drug Enforcement Administration (DEA) and authorized to prescribe controlled substances, complete a minimum of two (2) hours of Board-approved continuing education related to the safe and effective prescribing of controlled substances pursuant to section 461.007(3), Florida Statutes.
- Complete a minimum of one (1) hour of Board-approved continuing education related to human trafficking pursuant to Rule 64B18-17.001(8), F.A.C.
- Complete a two (2)-hour Board-approved course on prevention of medical errors pursuant to section 456.013(7), Florida Statutes.
- Complete a minimum of one (1) hour of Board-approved continuing education related to HIV/AIDS.
The practitioner is then exempt from any other continuing education requirements for his or her first renewal.

Helpful Information – Pro Bono Service
- Pursuant to s. 766.1115, Florida Statutes, for active licensees in good standing, up to ten (10) hours of pro bono service may be counted toward continuing education under the following conditions:
- Service must have taken place in the biennium in which the service was provided.
- Service must be performed under the auspices of a nonprofit agency or in clinics serving the indigent or otherwise underserved populations in areas of critical need within the state.
- Provides proof of such service submitted on the agency’s or clinic’s letterhead, with dates of service, number of hours, and signed by the director of the agency or clinic.
- Pursuant to s. 766.1116, Florida Statutes, for active licensees, when any health care practitioner:
- Participates as a health care provider under s. 775.1115, F.S.
- Provides services without compensation and as an agent of the governmental contractor to low-income recipients in accordance with s. 766.1115, F.S.
- Provides at least 160 hours of service during the biennium period for active licensees, or, if retired, provides at least 800 hours during the biennium period.
- Provides proof of such service submitted on the agency’s or clinic’s letterhead, with dates of service, number of hours, and signed by the director of the agency or clinic.
- Once requirements have been met, the licensee shall be eligible for:
- Waiver of the biennial license renewal fee for an active license; and
- Fulfillment of a maximum of 25 percent of the continuing education hours required for license renewal under s. 456.013(9).
- Pursuant to s. 381.00593, Florida Statutes, any health care practitioner who provides services:
- Without compensation
- In a public school for at least 80 hours a year for each school year during the biennial licensure period, or,
- If the health care practitioner is retired, for at least 400 hours a year for each school year during the licensure period.
- Provides proof of such service submitted by the applicable school district on the district or school letterhead, with dates of service, number of hours, and signed by the director of the school or school administrator.
- Once requirements have been met, shall be eligible for the following:
- Waiver of the biennial license renewal fee for an active license; and
- Fulfillment of a maximum of 25 percent of the continuing education hours required for license renewal under s. 456.013(9).
