Requirements
General Licensing Requirements

Podiatric Physician license requirements are specified in section 461.006, Florida Statutes.

Examination Eligibility Requirements:

Minimum Age Requirement: The applicant must be at least 18 years of age.

Application Completion:  The applicant must complete the application for licensure by accurately answering all questions and attaching all required supporting documentation.

Read Affirmation and Sign Application: The applicant must affirm by signing the application that: “I have carefully read the questions in the foregoing application and have answered them completely, without reservations of any kind, and I declare under penalty of s. 461.012(2)(b), Florida Statutes, that my answers and all statements made by me herein are true and correct. Should I furnish any false information in this application, I hereby agree that such act shall constitute cause for denial, suspension or revocation of my license to practice Podiatric Medicine in the State of Florida.”  

Background Screening:  The applicant must complete a criminal background screening pursuant to section 456.0135, Florida Statutes, by submitting fingerprints electronically through a service provider approved by the Florida Department of Law Enforcement.  For more information or to locate a live scan provider visit: https://www.flhealthsource.gov/background-screening

Education Requirements: The applicant must have received a degree from a school or college of podiatric medicine or chiropody recognized and approved by the Council on Podiatry Education of the American Podiatric Medical Association. The course of study shall be at least 4 years or the total hourly equivalent of a 4-year course of study.

Clinical Experience Requirements: The applicant must have completed one of the following clinical experience requirements:

  • One year of residency in a residency program approved by the board, and if it has been 4 or more years since the completion of that residency, active licensed practice of podiatric medicine in another jurisdiction for at least 2 of the immediately preceding 4 years, or successful completion of a board-approved postgraduate program or board-approved course within the year preceding the filing of the application.

OR

  • Ten years of continuous, active licensed practice of podiatric medicine in another state immediately preceding the submission of the application and completion of at least the same continuing educational requirements during those 10 years as are required of podiatric physicians licensed in this state.

Podiatric Medicine License Eligibility Requirements:

Examination Completion: Applicants must complete and pass the following examinations to be eligible for licensure:

Part I (Basic Science – General Anatomy)

Part II (Written Examination and the (CSPE) Clinical Skills Patient Encounter Examination)

Part III (Clinical Skills) – Eligibility Process

Residents must pass the Part III examination, and complete at least one year of residency before they can receive a podiatric physician license.

Before the state of Florida can deem an applicant eligible to take the Part III examination, each applicant must submit a “Podiatric Physician Application for Licensure and Examination” application, along with the associated fees. Complete the application in its entirety and mail it to the PO Box address on the application, along with the associated fee.

Once the application has been deemed complete, the board office will issue a letter of examination eligibility to take the Part III examination through APMLE. Applicants may then apply with AMPLE to take the examination and receive authorization from the Department.

Once the board office receives passing examination results, and the resident has completed one year of residency, a license will be issued.

Dispensing Practitioner Registration

Section 465.0276, Florida Statutes, requires that licensees, who dispense medicinal drugs for a fee or remuneration of any kind, whether direct or indirect in the State of Florida, shall be required to register with the Board and pay a fee of $100.

Dispensing is defined as the transfer of possession of medicinal drugs from a physician to a patient in the office. A practitioner who writes prescriptions or provides medicinal drugs labeled as “drug sample” or “complimentary drug” is not a “dispensing practitioner.” Practitioners who restrict activities to the dispensing of complimentary packages of medicinal drugs to patients in the regular course of medical practice shall not be required to register.

Dispensing Practitioners are required to comply with all laws and rules applicable to pharmacists and pharmacies, including, but not limited to:  Chapter 465, Florida Statutes (Pharmacy Practice Act), Chapter 499, Florida Statutes (Florida Drug and Cosmetic Act), and Chapter 893, Florida Statutes (Controlled Substance Act), and all federal laws and federal regulations. Renewal of dispensing registration runs concurrent with the issued podiatric physician license.

Applicants with Health History

If a “yes” response is provided to any of the health history questions on the application, the applicant is required to submit supporting documentation including the relevant dates and circumstances and the names and addresses of the medical practitioners or hospitals who performed related treatments. The application is considered incomplete until all supporting documentation is provided to the department.

Applicants with License Discipline History

Applicants with prior disciplinary actions on any license from any state are required to submit the following supplemental documentation:

Board Actions – Certified copies of documents relative to any disciplinary action taken against any license. The documents must be certified and submitted by the agency or board that finalized the disciplinary action.

Self-Explanation – Applicants who have listed license discipline on the application must submit a letter to the board personally describing the circumstances that led to the disciplinary action.  The letter should also include a thorough description of the rehabilitation taken by the applicant since the time of the event which would facilitate prevention of future occurrences.

Applicants with Criminal History

Applicants who have been convicted of a misdemeanor or felony; or entered a plea of guilty, nolo contendere, or no contest to any crime in any jurisdiction; even if adjudication was withheld by the court; must answer the criminal history questions on the application and provide the following supplemental documentation:

Final Dispositions/Arrest Records – Final disposition records for offenses can be obtained at the clerk of the court in the arresting jurisdiction. If the records are not available, a letter on court letterhead must be sent from the clerk of the court attesting to records unavailability.

Completion of Sanctions – Probation and financial sanction records for offenses can be obtained at the clerk of the court in the arresting jurisdiction. Parole records for offenses can be obtained from the Department of Corrections or at the clerk of the court in the arresting jurisdiction. If the records are not available, a letter on court letterhead must be sent from the clerk of the court attesting to records unavailability.

Self-Explanation – Applicants who have listed offenses on the application must submit a letter personally describing the circumstances of the offense. The letter must include the date of the original offense, the charge, and the jurisdiction where the incident occurred.

Electronic Fingerprinting - Background Screening

The applicant must complete a criminal background screening pursuant to section 456.0135, Florida Statutes, by submitting fingerprints electronically through a service provider approved by the Florida Department of Law Enforcement.  The results will be returned to the Care Provider Background Screening Clearinghouse and are made available to the Department for review during the licensure process. Applicant fingerprints will be retained by FDLE and the Clearinghouse. All costs for conducting criminal history background screening and the retention of fingerprints are the responsibility of the applicant.

It is important to use the correct Originating Agency Identification (ORI) when submitting fingerprints. If an incorrect ORI number is provided to the Livescan service provider, the Department will not receive the results.

The ORI number for the Board of Podiatric Medicine is EDOH2017Z.

The applicant is responsible for selecting an approved Livescan Service Provider and for ensuring the results are reported to the Department. Print the electronic fingerprinting form, complete the information, and take the form to a Livescan Service Provider.  Upon completion of electronic fingerprinting, document the Transaction Control Number. This number is useful to track the fingerprint records in the event they are not properly transmitted to the Department.

Health Care Fraud; Disqualifications for License, Certificate, or Registration

Effective July 1, 2012, Section 456.0635, Florida Statutes (F.S.), provides that health care boards or the department shall refuse to issue a license, certificate or registration and shall refuse to admit a candidate for examination if the applicant:

  1. Has been convicted of, or entered a plea of guilty or nolo contendere to, regardless of adjudication, a felony under Chapter 409, F.S., (relating to social and economic assistance), Chapter 817, F.S., (relating to fraudulent practices), Chapter 893, F.S., (relating to drug abuse prevention and control) or a similar felony offense(s) in another state or jurisdiction unless the candidate or applicant has successfully completed a drug court program for that felony and provides proof that the plea has been withdrawn or the charges have been dismissed. Any such conviction or plea shall exclude the applicant or candidate from licensure, examination, certification, or registration, unless the sentence and any subsequent period of probation for such conviction or plea ended:
    1. For the felonies of the first or second degree, more than 15 years from the date of the plea, sentence and completion of any subsequent probation;
    2. For the felonies of the third degree, more than 10 years from the date of the plea, sentence and completion of any subsequent probation;
    3. For the felonies of the third degree under section 893.13(6)(a), F.S., more than five years from the date of the plea, sentence and completion of any subsequent probation;
  2. Has been convicted of, or entered a plea of guilty or nolo contendere to, regardless of adjudication, a felony under 21 U.S.C. ss. 801-970 (relating to controlled substances) or 42 U.S.C. ss. 1395-1396 (relating to public health, welfare, Medicare and Medicaid issues), unless the sentence and any subsequent period of probation for such conviction or pleas ended more than 15 years prior to the date of the application;
  3. Has been terminated for cause from the Florida Medicaid program pursuant to section 409.913, F.S., unless the candidate or applicant has been in good standing with the Florida Medicaid program for the most recent five years;
  4. Has been terminated for cause, pursuant to the appeals procedures established by the state or Federal Government, from any other state Medicaid program, unless the candidate or applicant has been in good standing with a state Medicaid program for the most recent five years and the termination occurred at least 20 years before the date of the application;
  5. Is currently listed on the United States Department of Health and Human Services Office of Inspector General’s List of Excluded Individuals and Entities.

  • Online Application: Submit a completed application and the required fees to the Board office. Select the “Apply Online” button and follow the prompts to complete the application. The associated fee payment screen will provide an invoice that must be paid by credit card before the application can be processed. Once the application has been submitted, the status of application processing may be verified by selecting the “Status” button from the menu on the top right side of the page.
  • Paper Application: If submission of a paper application or payment by check is required, applicants may download a fillable pdf file, complete the application, print, and submit. Submit the completed paper application and required fees to the address listed below:

  • Examination Documentation: Contact the Federation of Podiatric Medical Boards (FPMB) for information on how to submit the American Podiatric Medical Licensing Examination (APMLE) scores to the Board office. Documentation is required to verify successful completion of Parts I, II and III.  Verification must be submitted directly to the Board office from FPMB.

Licensing By MOBILE Endorsement

Requirements

SB 1600, Mobile Endorsement was signed into law with an effective date of July 1, 2024. This legislation establishes a new universal process for licensure by endorsement for health care professions regulated by the Florida Department of Health’s (the Department’s) Division of Medical Quality Assurance. To qualify, the health care practitioner must meet specific criteria, including various testing, training, and experience qualifications for their profession. The bill also repeals existing licensure by endorsement statutes from various individual practice acts.

To be licensed as an optician in Florida through the MOBILE Endorsement pathway you must meet the following requirements in section 456.0145(2)(a), Florida Statutes:

  1. Must hold an active, unencumbered license issued by another state, the District of Columbia, or a territory of the United States in a profession with a similar scope of practice, determined by the board or the department, as applicable.
  2. Must have obtained a passing score on a national licensure examination or hold a national certification recognized by the Board;
  3. Must have actively practiced the profession for at least three years during the four-year period immediately preceding the date of submission of this application.
  4. Must not have ever been reported to the National Practitioner Data Bank, unless the applicant successfully appealed to have the report removed.
  5. Must not be the subject of a disciplinary proceeding in a jurisdiction in which he or she holds a license or by the United States Department of Defense for reason related to the practice of the profession for which the applicant is applying.
  6. Must not have had disciplinary action taken in the five years immediately preceding the date of submission of the application.

Pursuant to section 456.0145(2)(c), Florida Statutes, a person is ineligible for endorsement if they:

  • Have a complaint, an allegation, or an investigation pending before a licensing entity in another state, the District of Columbia, or a possession or territory of the United States;
  • Have been convicted of or pled nolo contendere to, regardless of adjudication, any felony or misdemeanor related to the practice of a health care profession;
  • Have had a health care provider license revoked or suspended by another state, the District of Columbia, or a territory of the United States, or have voluntarily surrendered any such license in lieu of having disciplinary action taken against the license; or
  • Has been reported to the National Practitioner Data Bank, unless the applicant has successfully appealed to have his or her name removed from the data bank.
Process

Applicants must complete the following steps:

  1. Have a license verification submitted to Florida from any state board(s), U.S. territory, or foreign country in which you held a license. If the state has an online verification including disciplinary actions, we will accept the online verification. License verification may be submitted by mail or by secure electronic delivery to MQA.PodiatricMedicine@flhealth.gov
  2. Submit proof of passing Part I, Part II, and Part III of the American Podiatric Medical Licensing Examination (APMLE). Proof must be sent directly from the Federation of Podiatric Medical Boards (FPMB) to this office, and may be submitted via mail or by secure electronic delivery to MQA.PodiatricMedicine@flhealth.gov
  3. Apply online at flhealthsource.gov, or mail a completed application to the address provided below along with the applicable fees, and submit any supplemental documentation.

    5. Health History – If the applicant answered in the affirmative to any of the questions, submit supporting documentation including the relevant dates and circumstances and the names and addresses of the medical practitioners or hospitals who performed related treatments.
  1. Please allow up to 30 business days for initial review of new applications. All applications and document submissions are reviewed in the order they are received. After your application is reviewed, any deficiencies will be communicated to you in writing by our Board staff. To expedite processing, please submit all required supporting documentation with your application and the requisite fees. If you are having documentation submitted by another entity on your behalf, please verify the Board office’s mailing address to ensure delivery.
  2. When all requirements are met, the license will be issued within 7 days pursuant to section 456.0145(2)(e), Florida Statutes.

Limited License for Podiatric Physician

Requirements

Current licenses expire at midnight, Eastern Time, on March 31, 2026

To ensure you receive your renewal notification from the department, your current mailing address must be on file. Failure to renew an active or inactive license by the expiration date will result in the license being placed in delinquent status. Failure by a delinquent licensee to renew before the expiration of the current licensure cycle renders the license null and void without any further action by the board or the department.

A licensee who remains on inactive status for more than two consecutive biennial licensure cycles and who wishes to reactivate the license may be required to demonstrate the competency to resume active practice by sitting for a special purpose examination or by completing other reactivation requirements.

Licensees requesting to reactivate their license from inactive or retired status are required to pay additional fees and comply with specific continuing education requirements. Please contact the board office at info@floridaspodiatricmedicine.gov to request your reactivation requirements.

Limited License Process

Renewal Instructions:

  1. Go to www.flhealthsource.gov and click the “Renew A License” button.
  2. Enter your Personal Identifying Information (PII), then click the Login button.
  3. Confirm or update your MQA Services Account email address, then click the “Continue” button.
  4. You are now in the Dashboard. During renewals, a “Renew My License” option will populate no later than 90 days prior to your license expiration date. (Note: If you do not see the “Renew My License” option, please check back at a later time.)

*** If the “PII Failed” screen displays, click the “Other Login Option” button and enter your existing User ID and password to access your account.***

  • You may renew online if you have a credit or debit card to complete the transaction. Please note that you can now renew online, print a copy of your application summary and mail it with your cashier’s check or money order to:

  • If you are renewing your license after the expiration date and your status still indicates Clear/Active, you will be assessed a delinquency fee. The delinquency fee will be in addition to your renewal fees.
  • If your license was Delinquent/Active before the renewal deadline, it will be moved to Null and Void status after the renewal deadline. You should contact your board office for further instruction.
  • The process of renewing a license and receiving a printed license in the mail may take 7-10 business days. Initiating contact to confirm the receipt of fees or the status of your license prior to this time will not expedite the renewal process.
  • Note: Do not submit certificates of completion of in-service hours to the Florida Board of Podiatric Medicine.
Name Change

If you are renewing online and need to change your name, please follow these instructions prior to renewing your license:

Name changes require legal documentation showing the name change. Please log into your MQA Online Services Portal account to submit your request and enter your full name as it appears on your legal documentation. If you are submitting your request online, please attach/upload supporting documents, which must be one of the following:

  • A copy of a state issued marriage license that includes the original signature and seal from the clerk of the court
  • A divorce decree showing the name change
  • A court order showing the name change (Adoption, legal name change, federal identity change)
  • A copy of a certificate of naturalization or H1B Employment Visa (Note: Foreign applicants and/or licensees may not have state issued documents)

Any one of these will be accepted unless the department has a question about the authenticity of the document.

NOTE: Please allow 5-7 business days for your request to be processed. If you are requesting a name change outside of renewal and wish to receive a new license reflecting the name change, please retrieve from the License Documents menu of your MQA Online Services Portal Account within two business days of the approval.

Requirements
General Requirements

Current licenses expire at midnight, Eastern Time, on March 31, 2026

To ensure you receive your renewal notification from the department, your current mailing address must be on file. Failure to renew an active or inactive license by the expiration date will result in the license being placed in delinquent status. Failure by a delinquent licensee to renew before the expiration of the current licensure cycle renders the license null and void without any further action by the board or the department.

A licensee who remains on inactive status for more than two consecutive biennial licensure cycles and who wishes to reactivate the license may be required to demonstrate the competency to resume active practice by sitting for a special purpose examination or by completing other reactivation requirements.

Licensees requesting to reactivate their license from inactive or retired status are required to pay additional fees and comply with specific continuing education requirements. Please contact the board office at info@floridaspodiatricmedicine.gov to request your reactivation requirements.

If you are reactivating your license, please refer to the Laws & Rules governing your practice for additional requirements.

The department will renew your license upon receipt of:

  • Completed renewal application
  • Required fees
  • Updated Practitioner Profile. To review and approve your practitioner profile, log into MQA Online Services and select ‘Review, Update & Confirm Profile’.
Military Exemption

If you are a member of the Armed Forces of the United States on active duty who was in good standing with the board at the time of becoming active and was entitled to practice or engage in the practice of medicine in Florida, you may be eligible to be exempted from renewal requirements for the duration of active duty, and for a period of six months after discharge, but not practicing the profession in the private sector for profit.

If you are the spouse of a member of the Armed Forces of the United States, you may be eligible to be exempted from licensure renewal requirements, but only in cases of absence from the state because your spouses’ duties with the Armed Forces.

Volunteer Health Services Exemption

If you volunteer your services to the medically indigent residents of Florida with incomes at or below 200% of the Federal Poverty Level, you may be eligible for a waiver of your biennial renewal fee and some of the continuing education hours (no more than 25%) required for license renewal; however, the continuing education requirements for domestic violence, HIV and AIDS, and medical errors cannot be waived.

Public School Health Volunteer Exemption

If you volunteer your services in public schools for a minimum of 80 hours a year or if retired, at least 400 hours a year, you may be eligible for a waiver of your biennial renewal fee and some of the continuing education hours (no more than 25%) required for license renewal; however, the continuing education requirements for domestic violence, HIV and AIDS, and medical errors cannot be waived.

Process
Renewal Process

Renewal Instructions:

  1. Go to www.flhealthsource.gov and click the “Renew A License” button.
  2. Enter your Personal Identifying Information (PII), then click the Login button.
  3. Confirm or update your MQA Services Account email address, then click the “Continue” button.
  4. You are now in the Dashboard. During renewals, a “Renew My License” option will populate no later than 90 days prior to your license expiration date. (Note: If you do not see the “Renew My License” option, please check back at a later time.)

*** If the “PII Failed” screen displays, click the “Other Login Option” button and enter your existing User ID and password to access your account.***

Additional Information:

  • You may renew online if you have a credit or debit card to complete the transaction. Please note that you can now renew online, print a copy of your application summary and mail it with your cashier’s check or money order to:

  • If you are renewing your license after the expiration date and your status still indicates Clear/Active, you will be assessed a delinquency fee. The delinquency fee will be in addition to your renewal fees.
  • If your license was Delinquent/Active before the renewal deadline, it will be moved to Null and Void status after the renewal deadline. You should contact your board office for further instruction.
  • The process of renewing a license may take 7-10 business days. Initiating contact to confirm the receipt of fees or the status of your license prior to this time will not expedite the renewal process.
  • Note: Do not submit certificates of completion of in-service hours to the Florida Board of Podiatric Medicine.
Name Change

If you are renewing online and need to change your name, please follow these instructions prior to renewing your license:

Name changes require legal documentation showing the name change. Please log into your MQA Online Services Portal account to submit your request and enter your full name as it appears on your legal documentation. If you are submitting your request online, please attach/upload supporting documents, which must be one of the following:

  • A copy of a state issued marriage license that includes the original signature and seal from the clerk of the court
  • A divorce decree showing the name change
  • A court order showing the name change (Adoption, legal name change, federal identity change)
  • A copy of a certificate of naturalization or H1B Employment Visa (Note: Foreign applicants and/or licensees may not have state issued documents)

Any one of these will be accepted unless the department has a question about the authenticity of the document.

NOTE: Please allow 5-7 business days for your request to be processed. If you are requesting a name change outside of renewal and wish to receive a new license reflecting the name change, please retrieve from the License Documents menu of your MQA Online Services Portal Account within two business days of the approval.

Licenses become Null & Void1 on April 1, 2026.

By submitting the appropriate renewal fees to the department, a licensee acknowledges compliance with all requirements for renewal, including continuing education.

First Biennium Renewal: Pursuant to s. 64B18-17.005, F.A.C., Podiatric Physicians initially licensed in the biennium or within 24 months of initial licensure are required to:

  • Obtain five (5) hours of continuing education in the subject area of risk management. This may be obtained by attending one full day of a meeting of the Board of Podiatric Medicine at which disciplinary hearings are conducted, or it may be obtained by taking five (5) hours of board approved courses.
  • Take a minimum of two (2) hours of board approved continuing education related to the safe and effective prescribing of controlled substances pursuant to s. 461.007(3), F.S.,
  • Take a minimum of one (1) hour of board approved course related to human trafficking pursuant to s. 64B18-17.001(8), F.A.C.
  • Take a two (2) hour board approved course on prevention of medical errors pursuant to 456.013(7), F.S.
  • Take a minimum of one (1) hour of board approved courses related to HIV/AIDS.

The practitioner is then exempt from any other continuing education requirements for his or her first renewal.

Helpful Information – Pro Bono Service

  • Pursuant to s. 766.1115, Florida Statutes, for active licensees in good standing, up to ten (10) hours of pro bono service may be counted toward continuing education under the following conditions:
    • Service must have taken place in the biennium in which the service was provided.
    • Service must be performed under the auspices of a nonprofit agency or in clinics serving the indigent or otherwise underserved populations in areas of critical need within the state.
    • Provides proof of such service submitted on the agency’s or clinic’s letterhead, with dates of service, number of hours, and signed by the director of the agency or clinic.
  • Pursuant to s. 766.1116, Florida Statutes, for active licensees, when any health care practitioner:
    • Participates as a health care provider under s. 775.1115, F.S.
    • Provides services without compensation and as an agent of the governmental contractor to low-income recipients in accordance with s. 766.1115, F.S.
    • Provides at least 160 hours of service during the biennium period for active licensees, or, if retired, provides at least 800 hours during the biennium period.
    • Provides proof of such service submitted on the agency’s or clinic’s letterhead, with dates of service, number of hours, and signed by the director of the agency or clinic.
    • Once requirements have been met, the licensee shall be eligible for:
      • Waiver of the biennial license renewal fee for an active license; and
      • Fulfillment of a maximum of 25 percent of the continuing education hours required for license renewal under s. 456.013(9).
  • Pursuant to s. 381.00593, Florida Statutes, any health care practitioner who provides services:
    • Without compensation
    • In a public school for at least 80 hours a year for each school year during the biennial licensure period, or,
    • If the health care practitioner is retired, for at least 400 hours a year for each school year during the licensure period.
    • Provides proof of such service submitted by the applicable school district on the district or school letterhead, with dates of service, number of hours, and signed by the director of the school or school administrator.
    • Once requirements have been met, shall be eligible for the following:
      • Waiver of the biennial license renewal fee for an active license; and
      • Fulfillment of a maximum of 25 percent of the continuing education hours required for license renewal under s. 456.013(9).
