Attention: The Department of Health, Division of Medical Quality Assurance will be upgrading its licensing and enforcement system from Thursday, November 13th at 6:00 PM (EST) to Monday, November 17th at 8:00 AM. Our online services will not be available because of this system upgrade. We apologize for any inconvenience this may cause.
